Make sure you have the correct identification before taking a HS&E (CSCS) test. Please read the CITB ID Policy for a complete guide on ID requirements. Original documents (not copies) are required for proof.

Using a Waiver Form.

If you have problems with proof of ID, you can complete the Waiver Form.
This form must be completed, with two references, before the HS&E Test (CSCS).
